Community Based Services

ADRC (Aging and Disability Resource Center)

The Aging and Disability Resource Center (ADRC) provides current information on services, benefits, and programs that are available to assist citizens, their families, and interested persons of all ages. The ADRC specialist will screen for eligibility for public and community benefits and services and will help with the application process. CAAC is also linked to a statewide network with access to information on services available anywhere in Alabama and on a limited basis, nationwide. CALL (334) 240-4670.

Dementia Friendly Communities

CAAC works with community partners, caregivers and those with dementia to foster “dementia friendly” communities across the state to improve opportunities and outcomes for a growing number of people with dementia, their families, and care providers. This program promotes education, awareness, services, and resources to improve quality of life.

Nutrition

CAAC sponsors senior centers located in various community facilities throughout the tri-county area where seniors gather for lunch and enjoy recreational, educational, physical, and social activities. Home delivered lunches are available to home-bound individuals age 60 and older who are unable to attend a senior center.

Nutrition Counseling

Nutrition counseling is available on a limited basis to individuals 60 and older who are identified as at-risk for poor nutrition. A licensed nutritionist meets individually with clients to assist them in developing a healthy nutrition plan.

SenioRx

The SenioRx Program assists uninsured and underinsured individuals with obtaining medications at a reduced rate or no cost. Medication management is also available.

SHIP

The State Health Insurance Assistance Program (SHIP) counselors assist individuals with questions about Medicare, Medicaid, supplemental insurance, long-term insurance and related coverages. Counselors also provide information on how other insurances work with Medicare, assist with applications for Medicare related programs, and provide educational materials and programs to the community.
